To make an appointment or set up an initial consultation, please call our office at (231) 924-5309. Clinical appointments are established according to each therapist’s schedule. This may involve appointments during the day or evening.
We Offer Consultation
If you don’t fully understand a mental health related situation or are trying to figure out how to proceed, consultation may be beneficial. Contact our office if you would like one of our staff to provide consultation regarding specific questions or issues. Consultation may involve an individual or group.
We often consult with various agencies, such as court systems, attorneys, schools, churches, businesses, mental health facilities and medical personnel. This may sometimes involve specific trainings, seminars and/or educational presentations.
Insurance and Billing
Most insurance companies have provisions for psychological services. Clients are responsible to communicate with their insurance company to verify coverage, co-pays and deductibles.
We accept self-pay for those without insurance, or those who choose not to use their insurance. Patients are encouraged to discuss financial arrangements confidentially with their therapist.
We accept MasterCard, Visa & Discovery credit cards, and Health Savings Account (HSA) cards for payment of co-pays, deductibles and/or private pay.
New Patient Forms
If you are a new patient, we recommend arriving at least fifteen minutes early to your first appointment to allow time to start the intake process and complete the necessary forms. To save time, you may print out adult or child forms by clicking on the corresponding link below, fill out the appropriate form(s) at home and then bring it to our office at your scheduled appointment time. If you have questions as to what forms are necessary, please call our business office.
